卵円孔静脈叢 Plexus venosus foraminis ovalis

解剖学的構造と位置関係

• 卵円孔静脈叢は、頭蓋底の卵円孔を通過する静脈性血管網で、下顎神経（V3）に随伴する (Rhoton, 2020)。
• 海綿静脈洞の後外側部と翼突筋静脈叢を連結する、重要な静脈経路を形成する (Gray et al., 2023)。
• 周囲の重要構造物：
  • 内側：内頸動脈
  • 外側：中硬膜動脈
  • 前方：上顎神経（V2）

機能と血行動態

• 頭蓋内外の静脈血の交通路として機能する (Tubbs et al., 2022)。
  • 海綿静脈洞からの静脈血を翼突筋静脈叢へ導く。
  • 頭蓋内圧調節に寄与する側副循環路の一部を形成する。

臨床的意義

• 頭蓋底手術における重要な解剖学的指標となる (Osborn, 2021)。
• 腫瘍の頭蓋外進展経路となる可能性があり、注意を要する (Salma et al., 2024)。
• 静脈性出血のリスクが高いため、手術時には特に慎重な操作が必要である。
• 硬膜動静脈瘻の好発部位の一つとして広く知られている (Berenstein et al., 2023)。
