前有孔質枝（前脈絡叢動脈の） Rami substantiae perforatae anterioris arteriae choroideae anterioris

解剖学的特徴

• 前脈絡叢動脈から分岐する穿通枝で、前有孔質を貫通して内側に向かう (Rhoton, 2002)。
• 主に内包後脚、淡蒼球外節、視床の一部を灌流する (Marinković et al., 2001)。
• 直径は約0.1-0.3mmで、通常2-4本の穿通枝が存在する (Fernández-Miranda et al., 2012)。

血管支配領域

• 内包後脚：錐体路や視放線などの重要な神経線維束を含む (Wen et al., 2009)。
• 淡蒼球外節：運動制御に関与する大脳基底核の一部 (Erdem et al., 1993)。
• 視床後外側部：感覚情報の中継に重要な領域 (Türe et al., 2000)。

臨床的意義

• これらの血管の閉塞は、片麻痺、感覚障害、視野欠損などの重篤な神経症状を引き起こす (Morcos and Yasargil, 2014)。
• 高血圧性脳出血の好発部位の一つであり、微小動脈瘤の形成にも注意が必要である (Kumral et al., 2004)。
• 脳動脈瘤の手術時には、これらの穿通枝の温存が極めて重要である (Lawton, 2011)。
