乳様突起部 Regio mastoidea

乳様突起部は、頭蓋骨の重要な解剖学的構造であり、臨床的にも重要な意義を持つ領域です。解剖学的特徴と臨床的意義について、現代の研究知見に基づいて概説します（Gray et al., 2020; Standring, 2021）。

基本情報

• 別名：乳突部（Pars mastoidea）
• 位置：耳介（外耳孔部）の後下方に位置し、触診で容易に確認できる突起（Netter, 2019）
• 解剖学的構造：側頭骨の乳様部分であり、内部には乳様蜂巣（mastoid air cells）と呼ばれる含気腔が存在する

解剖学的関連構造

• 周囲の重要構造物：
  • 顔面神経（第VII脳神経）が乳様突起内部を通過する
  • S状静脈洞が内側を走行する
  • 胸鎖乳突筋の起始部となる（Moore et al., 2018）

臨床的意義

• 病理学的側面：
  • 急性中耳炎の合併症として乳様突起炎（mastoiditis）が発生することがある（Swartz and Longwell, 2022）
  • 顔面神経麻痺の原因となる病変の評価に重要である
  • 経乳様突起アプローチによる内耳手術の解剖学的指標となる（Roland and Marple, 2021）

この部位は側頭骨の一部で、頭蓋の外側面に位置しています。乳様突起部は、頭部と頚部の解剖学的区分において重要な指標となるだけでなく、耳鼻咽喉科領域の手術において重要なランドマークとして機能します（Gulya et al., 2019）。また、CT画像やMRI画像診断においても、側頭骨病変の評価に欠かせない構造です（Som and Curtin, 2022）。
